I did an eye exam, MD visit, wellness survey and financial survey but only got 2 credits what do I do.
Theresa – It takes up to two months for the actions to get loaded into your wellness summary. Have you given it enough time?
I can’t seem to update surgeries, doctor’s visits, or exams. What can I do?
Hi Linda – What do you mean when you say update? You have to actually make a visit to a healthcare provider, complete a screening, view the financial wellness video, complete the GAP Registry, and Health Survey in 2017 to receive the credits in 2018. Hope that helps.
Where do I find my completed wellness pledges ( the MD appointments such as yearly dental, physical, mammo, eye).
Pat – The wellness actions take about six to eight weeks to appear in your wellness summary. To view your wellness summary – login to mySelfService and click on the myBenefits tab. Scroll to the bottom and you will see your wellness summary with your date of service.
For team members to receive all credits beginning with their first paycheck in January 2018, is the recommended completion date for all Wellness Pledges October 31, 2017?
Yes Michael that is correct. To earn your maximum amount of 2018 paycheck credits ($1,040) complete four healthy actions before October 31.
From UHC to Careconnect, I registered to Rally when I still have UHC. Do I need to sign up again since I have new Insurance?
S.Salvacion – If you are insured through CareConnect in 2017, you will need to complete the survey at careconnect.com/nwhealthsurvey
I had my annual papsmear done…how to report this apt to receive credit?
Hi Anna – Yes the cervical cancer screening (also known as a Pap Smear) is one of the 10 Wellness Actions if completed will earn you paycheck credits in 2018. Be sure to complete the online Health Survey before October 31, 2017. The survey is a necessary action to earn any of the credits.
Is there a place to submit a hard copy proving the visit for the healthcare credits, especially if it has been 6-8 weeks?
Rachel – scan and attached to an email to benfits@northwell.edu along with a note.
I had a dental visit and cleaning done quite a while ago-past 8 weeks but I still do not see that I have gotten credit for it. What can I do to receive credit?
Melinda – I would give it another week or two, or you could call your dentist office and request the date your information was sent to Cigna.
I can’t find the Health Survey. I have UHC and when I log into the myuhc website, it’s a very stark dashboard with 3 links to find a doctor, manage claims, and view saved doctors and facilities. I scroll down and there is info on costs, claims etc. I have been searching everywhere on all the menus and can’t find the survey that is required to complete. This happens every year – there is no clear communication on how to get to the actual survey.
Hi Jen – Thanks for checking in. You are correct the Health Survey is currently unavailable, but should be back up in a day or so.
Can you complete the financial wellness pledge each year for credit or only once?
Hi Carmine – You can receive credit every year for watching the video. A new video is currently in production.
I attempted to access the Financial Wellness Pledge Video and the link didn’t open the program. Please advise.
Rosanne – We are checking into this. Thank you!
